Online Appointment Request Procedure

Please complete the intake form below and fax it to us at (985) 624-8643. You will receive a return call from our intake coordinator within 1 business day to discuss your intake, our policies and to schedule an appointment.

Your First Appointment

Please call the office and speak with our intake staff. After receiving your consent form which our staff will explain, you will be given instructions on completing the intake process on line.

Please go to www.valant.io/myio/centeradhd (Your pop-up blocker may need to be disabled or modified to utilize this link) on your computer or iPad to register as a new patient. You will need to complete on all forms on line as instructed by the portal. These forms and the information they gather will be recorded in the electronic medical record system we use to coordinate and document your care. Please call the office of you have any problems with this process.

It is essential that all these forms be completed prior to your first appointment. Your appointment will be rescheduled if they are not completed prior to the scheduled time of your first visit. If you don't have access to a computer or the internet, please arrive 1 hour early and we will gladly supply you with an iPad to complete the forms in our waiting room.

Insurance and Payment Information

Dr. Brown and The Center for ADHD are providers for several major insurance plans, and provides insurance billing. You are responsible for co-payments or charges that are not covered by your insurance. If you have questions regarding billing or which insurance plans we accept, please contact us. Questions regarding insurance coverage and benefits should be directed to your employer or insurance company.

Privacy Practices

Our Notice of Privacy Practices describes how medical information about you may be used and disclosed and how you can get access to this information. It is given to all new patients, and a copy is available upon request.
